When will I get a paper statement?

How do I get a loan from my retirement account?
If your plan allows loans, you can request a loan in one of two ways.
You can call Client Plan Services at 1-800-876-9557 for assistance.
You can request a loan by logging into your personal account in Start Right Online login. Once you have logged into your account, click on the drop down arrow on the left navigational panel and select Account Management, then select Loans. In the middle of the page you will see Loan Type. The most common selection is General Purpose.
BE SURE TO KEEP YOUR CONFIRMATION NUMBER WHEN THE LOAN IS REQUESTED THROUGH YOUR PERSONAL ACCOUNT.

How do I change my contribution elections?
To change your contribution elections, print the Enrollment Form. Once you have completed the form to reflect the changes you desire, give a copy to your Office Manager, who will then fax it to UPAL at (918) 747-5596.
How do I change my beneficiary designations?
To change your beneficiary designations, print the Beneficiary Designation Form. After you have completed the form to reflect the changes you desire, give a copy to your Office Manager, who will then fax it to UPAL at (918) 747-5596.
How do I change my investment allocations?
Changes can be requested through Bank of Oklahoma by calling Voice Direct (1-800-686-9457), or online at www.startright.bokf.com. Print and retain the Investment Asset Allocation Worksheet for your records.
